- An image of your valid US government-issued photo identification, such as:
- State Driver’s License,
- Passport,
- or other signed, current, photo identification
For both a name change, update or correction or a social security number update/correction, you must also upload an image of your:
- Social Security Card (for both a name change, update or correction or a social security number update/correction)
