Inactive/Expired CNA License Renewal Application

Please attach and upload proof of working 8 hours in nursing services two years prior to your inactive or expired date and every two years thereafter.  Examples of nursing services includes direct care tasks such as dressing, toileting and transfers,etc.  Proof of work may include paystubs, W2’s, or a letter from your employer on the company letter head.  If you have recently went inactive, within the last 24 months, you may click the link for instructions to complete the renewal directly from your profile if you log into TMU.

 Acceptable documents: 
  • Paystub
  • W-2
  • Letter from employer, signed on letterhead
  • If working for a Staffing Agency - work schedule or pay summary
  • If working in private duty - letter from the caregiver that includes patient name, CNA name and certificate number, dates worked/paid for, work done (give a brief description of work done - CMT/Med Assistant qualifies as well), signature of both patient and CNA, dated.
